The header, top portion of the report, contains the address of the sample submitter that was written on your request sheet. The header also contains the date we received (checked in) your sample as well as the date the report was finalized and printed. The most important information on the right side is the Report Number and the Lab Number. Should you need to contact us regarding your samples/report, these number will allow us to easily search your test information in our database.
